The Nutrition Circle will, from time to time, receive and store personal information you enter onto our website, provided to us directly or given to us in other forms, including through patient forms and during appointments. When you register as a patient of our practice, you provide consent for The Nutrition Circle and practice staff to access and use your personal information so we can provide you with the best possible healthcare.
  • 2.2. The information we will collect about you includes your name, date of birth, address, phone number, email address, health information including your medical history, medications, allergies, adverse events, immunisations, social history, family history and risk factors, Medicare number, healthcare identifiers, health fund details to enable us to provide you with the best healthcare services, to send information and provide updates to you
  • 2.3. You may provide basic information such as your name, phone number, address and email address to enable us to send information, provide updates and provide the services. We may collect additional information at other times, including but not limited to, when you provide feedback, when you provide information about your health, personal or business affairs, change your content or email preference, respond to surveys and/or promotions, provide financial or credit card information, or communicate with our customer support.Although the Privacy Act requires us to provide you with the option of not interacting with us anonymously or with a pseudonym, we note it is impracticable for us to provide you with healthcare services if you interact with us anonymously or with a pseudonym.

  • 7.3.  We reserve the right to refuse to provide you with information that we hold about you, in certain circumstances set out in the Privacy Act.
